Phone Shutsdown At "... Just A Sec" During Welcome Setup

Beginning with a stock tmobile HTC sensation, I followed the procedures at:

and everything seemed to work fine but when starting from the cyanogen Welcome page, I tap the Start button, get "Just a sec..", the animated line stops and the device shuts off.  I've reformatted the sd card, cleared all the caches, used different downloads of cm-9.1.0-pyramid.zip and attempted both with & without the google apps.  All have the same result.

The adb logcat seems to always end with a profileService save:

I saw this thread forum.xda-developers.com/showthread.php?t=1668276 for a "do-it-all" tool but I don't use windows.
Any ideas or recommendations?

The problem ended up being a mismatch of the firmware .  

The instructions I used for a TMO-US Sensation were here:

The final set of files used for all the steps were:
http://revolutionary.io (fill form with your device info) - gain S-OFF  but choose NO to install clockwork mod
recovery-clockwork- - clockwork mod recovery
PG58IMG.zip - patched TMO-US firmware/radio
cm-9.1.0-pyramid.zip - cyanogen 9.1
gapps-ics-20120317-signed.zip - google apps
su- - permanent root

Everything works perfect so far..
